Request for Transfer of Records
Often, we request the transfer of records from offices where you have a history. This is most helpful where there has been extensive treatment or if a significant series of events affected prior treatment outcomes. Knowing what was done, when it was done, and why it was done reveals knowledge that will prove valuable moving forward.
Requesting the transfer of your records can be accomplished simply by signing an authorization form at our office or one we can send you. Some people prefer to contact their previous dental office themselves to have their records sent, and some people will choose to forego this transfer entirely for personal reasons. We feel strongly that having access and understanding your history is beneficial on many levels.
A thorough review of your past medical and dental history will provide us with valuable insights about the current state of your oral health and specific dental concerns. As part of your review, an evaluation of the supporting bone and soft tissues is needed to make an individualized recommendation for treatments with one of our dental hygienists.
